Care Track is a complete Digital care-planning solution, with care-capture functionality and supporting features that can be used to enrich personal care plans whilst helping to evidence the care in place.

In community and care-based organisations, attention to detail is key to giving the best level of care. Organisations in these industries today are lacking a centralised system for evidencing their healthcare and support work during care of community projects; tracking customer records; and logging notes against customers and activities.

Our Care Track App is a responsive mobile solution which leverages the Microsoft Power Platform to deliver a responsive mobile solution for community and care-based organisations. Through this App, carers, community service delivery teams, support workers and clinical professionals can capture outcome-based evidence for their customers and beneficiaries in the form of pictures, videos, documents and notes against personalised care plans and goals.

Through Care Track it is possible to quickly identify a customer through search or QR code recognition, so that care and support teams can quickly start to work on their evidence captures and track progress with easy upload and tagging systems. Care Track is a complete Digital care-planning solution, with supporting features that can be used to enrich personal care plans whilst helping to evidence the care in place.

Care Track can be used as a standalone application, or as an embedded view within Microsoft Dynamics 365. The Dynamics 365 integration makes storing and transferring data completely seamless and gives organisations a centralised view of their care and support, at every level. And, it's built in according with the Common Data Model for Health, meaning you can be assured that the data captured is part of a wider common framework that has a roadmap, is industry-appropriate and can be adopted with other platforms.
